Requirements: Education - Doctorate Required, Field of Study: Board Certified DDS or DMD. Experience: Minimum 4 years in doctorate program with receipt of Board Certification as Doctor of Dental Surgery or Doctor of Medical Dentistry. Certifications & Licenses: Current license to practice as a Dentist in Oklahoma.
