The Application process will be open May 15, 2016 through September 1, 2016

for U.S. applicants only.

The Graduate Orthodontics Program will not be accepting applications for

international students again until 2018.


 

Admissions Application Checklist

 

  1. University of Oklahoma Health Sciences center online application form.  Fee applied.
  1. College and Dental School transcripts should be sent to the following address:

  1. Completed (typed) supplemental application form.  Download the supplemental application form (PDF Form – Click to download).  Download the supplemental application form, complete form on your computer, then print and send in the completed form with the other application materials. Remember to attach your 2 x 2 photo to the supplemental application.
  1. Three letters of recommendation (at least one from current/former professor).
  1. Official Documentation from Dean’s Office of current class rank and cumulative G.P.A.
  1. Official National Boards score.
  1. Curriculum Vitae (CV).
  1. Personal Statement.
